GLP-1 Telehealth Costs Compared (2026)
What you actually pay, including the medication, the program fee, and the extras a headline price can leave out.

Prices change often. We last checked on November 1, 2026; confirm with each provider.
What you’re actually paying for
Most programs split the bill. A simple way to compare them is:
Total monthly cost = medication + membership + any extras
Medication
This is the drug itself, and it’s usually the biggest part. The price depends on the medication, its form, and your dose, which your clinician sets.
Program or membership fee
This pays for access to clinicians and support. Most brand-name programs charge it on top of medication, so a low medication price doesn’t mean a low total.
Labs and supplies
Ro says lab testing is included if necessary. Elsewhere, ask whether labs cost extra. Tirzepatide’s KwikPen and vial also need pen needles or syringes, bought separately.
Insurance
If your plan covers the medication, your copay may replace most of the medication cost. You’d usually still pay any membership fee.
Manufacturer self-pay prices
These are cash prices from each manufacturer’s direct program, without a telehealth membership. You still need a valid prescription. Fills last 28 days for pens, vials, and KwikPens, and 30 days for pills.

Reading the table: Fees are ongoing rates, not introductory offers. “Brand” means the page we checked listed FDA-approved medication only. “Not listed” means the page we checked didn’t describe insurance help.
Found: “Not comparable” means its listed plan prices bundle compounded medication, so they can’t be compared with brand-name pricing. Found is left out of the quick answer range. See its pricing page for current plans.
Compounded options
Some programs also offer compounded GLP-1 products, often advertised at lower prices. We aren’t listing compounded program pricing for now.
Why prices can be lower
Compounded drugs don’t go through FDA approval, so they don’t carry the costs of that process. A lower price tells you nothing about safety, effectiveness, or quality.
FDA status
Compounded drugs are not FDA-approved, and they are not the same as FDA-approved products. The FDA doesn’t verify their safety, effectiveness, or quality before they’re marketed. Our side effects guide covers the concerns the FDA has reported.
Current FDA enforcement
- The FDA says semaglutide and tirzepatide aren’t on its drug shortage list.
- In February 2026, it announced plans to restrict the ingredients used in non-approved compounded GLP-1s, and said what companies can’t claim about them.
- In April 2026, it proposed excluding both from the list of substances outsourcing facilities may compound from in bulk. Comments ran through July 30, 2026, and we found no final decision as of November 1, 2026.
- In 2026, it sent warning letters over compounded GLP-1 marketing, including this June letter.
This means compounded options could change or become harder to find. If one is part of your budget, ask what happens if it stops being available.
Ways to lower cost
Check your insurance first
Ask whether the medication is covered and whether prior authorization is needed. Employer coverage varies. KFF found 18% of firms with 200 or more workers cover GLP-1s used primarily for weight loss in 2026.
Look at manufacturer programs
If your commercial plan covers the medication, savings cards may help. Lilly and Novo Nordisk both advertise paying as little as $25, with maximum savings limits. People in government programs such as Medicare or Medicaid are generally excluded. Read the terms, because these offers can’t usually be combined.
Consider a prepaid plan, carefully
Ro’s 12-month plan is $74 a month instead of $149 on its monthly plan, which saves $900 a year. Paying upfront only saves money if you stay on the program, so check the refund and cancellation terms first. A clinician may also change or stop your treatment.
Refill on time
Tirzepatide’s $449 price depends on refilling within 45 days. A late refill can cost up to $699. Orforglipron’s $299 price for its two highest doses has the same 45-day rule, and a late refill costs $349.
Use an HSA or FSA, if eligible
The IRS allows amounts paid to lose weight as a medical expense when it’s treatment for a specific disease diagnosed by a physician, such as obesity. It doesn’t allow them for appearance, general health, or a sense of well-being. HSA and FSA rules generally follow those medical expense rules. IRS guidance doesn’t name GLP-1s, so check with your plan administrator or a tax professional.
Budgeting over 12 months
Here’s a simple yearly example for someone early in their career. Doses are illustrative only, because your clinician sets your dose. Because each fill lasts 28 days, a year takes 13 fills, not 12.
| Medication | Medication math | Medication total | With $74/mo membership | With $149/mo membership |
|---|---|---|---|---|
| Semaglutide pen | Medication math2 × $199 + 11 × $349 | Medication total$4,237 | With $74/mo membership$5,125 | With $149/mo membership$6,025 |
| Tirzepatide vial or KwikPen | Medication math$299 + $399 + 11 × $449 | Medication total$5,637 | With $74/mo membership$6,525 | With $149/mo membership$7,425 |
Membership adds $888 a year at $74 a month, or $1,788 at $149. Semaglutide assumes the 2 introductory fills, then the $349 price. Tirzepatide assumes one fill each at 2.5 mg and 5 mg, then higher doses refilled on time. We left out first-month offers, taxes, and supplies.
- Per paycheck: With 26 paychecks a year, the lower totals work out to about $197 and $251 each.
- Plan for changes: A new job, new insurance, or a rent increase can change the math. Recheck your budget at each one.
- Plan the long term: These are long-term medications, and stopping has effects. Our under 35 guide covers both.
Frequently asked questions
How much does a GLP-1 cost without insurance?
Listed manufacturer self-pay prices we checked on November 1, 2026 run from $149 to $449 a month, depending on the medication, form, and dose. A late tirzepatide refill can cost more. A telehealth membership, if you use one, is usually a separate charge.
What is the cheapest GLP-1 telehealth program?
There isn’t one answer, because it depends on your medication, your dose, and how you pay. The lowest ongoing membership fee we found was $74 a month, and it requires a 12-month plan. Compare total cost, not just the membership fee.
Does a telehealth membership include the medication?
Usually not for brand-name medication. Ro, Hims & Hers, and WeightWatchers all say medication is billed separately, and Found says the same for brand-name medication. Check this before you pay.
Can I use insurance with a telehealth program?
Often, yes. Ro, Found, and WeightWatchers all describe insurance help, but coverage depends on your plan. Ask your insurer whether the medication is covered and whether it needs prior authorization.
Why are compounded options often advertised for less?
Compounded drugs don’t go through FDA approval, so they don’t carry those costs. They are not FDA-approved, and they are not the same as FDA-approved products. A lower price tells you nothing about safety, effectiveness, or quality.
